Most projects that have a required purchase will also require that the receipt be submitted with the mystery shop evaluation form. There are several ways to do this:
1) Scan and upload. You can scan the receipt, save it as a pdf or jpg file (NOT as a Word document), and then upload it into the evaluation form. (There is an “upload” button right on the form.)
2) Fax a copy to our office. You can print a fax cover sheet from a link on the evaluation form (found at the top in the Project Specifications). The cover sheet contains everything we need to know to identify the shop. Also, please fax the receipt “right side up” so we don’t have to stop and edit before attaching (every fax machine is different, so please check yours). Our fax number appears right on the cover sheet.
3) Email it to the scheduler. You can scan the receipt, save it as a pdf or jpg file (NOT as a Word document), and send it as an attachment on an email to the scheduler. (Please send it as an “attachment”, don’t just embed in the email.)
